CPP-HR 0706 Dependent Medical Check-Up Policy

CPP-HR 0706 Dependent Medical Check-Up Policy

Policy Statement:

Infinit-O Philippines Inc. is committed to promoting the health and well-being of its team members and their dependents. As part of this commitment, dependents may avail of medical consultations with the company’s onsite doctors, subject to the guidelines outlined below.

  1. Objective:

To establish clear guidelines for the scheduling, type, and conduct of medical consultations for team member dependents, ensuring a safe, efficient, and accessible process that aligns with Infinit-O’s health and safety protocols. This initiative also supports preventive care efforts and enhances team member well-being through accessible family health support.

  1. Scope:

This policy applies to all active team members of Infinit-O who wish to request medical consultations for their qualified dependents through the company’s onsite doctors. It covers both online and onsite consultations and outlines the procedures, limitations, and coordination requirements involved.

Only dependents enrolled under the team member’s current HMO provider are eligible for consultation. Proof of HMO enrollment may be required during booking or prior to the consultation.

This policy applies to all enrolled dependents. Online consultation is available for all, however due to building restrictions and adherence to safety protocols, physical consultation is only available for dependents 18 years old and above. Physical consultations still remain available in accredited clinics/hospitals by the HMO provider.

  1. Provisions:

To ensure the safety of all individuals within company premises and to maintain operational efficiency, the following policy applies:

  1. Type of Consultation

    1. Online consultations are required for dependents showing symptoms of illness (e.g., fever, cough, rashes). This is to prevent the potential spread of infection within the office.

    2. Onsite consultations are allowed only for dependents seeking general or preventive health check-ups and are not exhibiting any symptoms. Please note that the onsite consultation schedule is subject to change depending on the availability of the attending doctor.

  1. Prior Booking

    1. All consultations, whether online or onsite, must be scheduled in advance through the appropriate booking channel.

    2. Walk-in appointments will not be accepted.

  1. Access Restrictions

    1. Minor dependents are generally not allowed inside office premises. If the principal employee wishes to have their child dependent seen by a doctor, they may be referred to another physician accredited by our HMO provider.

  2. Scope of Onsite Medical Services

    1. Not all medical concerns may be addressed onsite. Team members are advised to coordinate ahead with the IO Doctor to confirm whether the dependent’s condition can be accommodated.

    2. Cases that fall outside the scope of their services may be referred to external providers accredited by company HMO provider.

  1. Emergency Cases: Company doctors will not accommodate emergency cases. In such instances, team members must bring their dependents to the nearest accredited clinic or hospital covered by their HMO provider for immediate care.

  1. Consultation Protocol

    1. For online consultations, dependents must be available at the scheduled time via the designated virtual platform.

    2. For onsite consultations, the dependent must follow all entry and health protocols, and must be accompanied by the team member or an authorized guardian.

    3. All dependent onsite consultations must be pre-booked through the designated scheduling platform. Walk-in consultations are strictly not allowed, and no entry will be permitted without a confirmed booking. This is to ensure proper coordination with the onsite doctor, verification of dependent eligibility, and compliance with safety and security protocols.

  1. Responsibility:

  1. HR ER

    1. HR-ER is responsible for communicating the guidelines and any updates regarding dependent medical consultations to all team members via email and or internal online platform.

  1. Coordinates with the Infinit-O doctors and SSP team for onsite consultation requests.

  1. Verifies the HMO enrollment status of dependents prior to confirming consultation schedules, if needed.

  1. Provides the updated list of enrolled team members and their eligible dependents to the Infinit-O doctor to assist in verifying the relationship between the team member and the dependent.

  1. Ensures that the implementation of this policy aligns with company's health, safety, and data privacy standards.

  1. Team member

    1. Ensure that their dependents are enrolled under the current HMO provider to be eligible for consultation services.

  1. Submit a request and secure a confirmed booking for their dependent’s consultation (onsite or online) through the designated channel.

  1. Attend the dependent's scheduled consultation on time. If unable to do so, inform the company doctor ahead of time to avoid any implications.

  1. Adhere to the policy that onsite consultations are only for preventive/general check-ups and that children are not allowed inside the office premises.

  1. Seek immediate care for emergency cases through the nearest HMO-accredited clinic or hospital, as emergency consultations are not handled by the company doctor.

  2. Maintain proper coordination with HR-ER for any concerns, clarification, or follow-up related to the dependent consultation process.

  3. If a dependent is unable to attend a scheduled online or onsite consultation, the team member is expected to inform the company doctor in advance. Failure to provide notice may result in a corresponding salary deduction.

  1. Infinit-O Doctor

    1. The onsite doctor is responsible for reviewing consultation requests and confirming whether the medical concern falls within the scope of services provided onsite.
