Card Replacement Request
A. If request is due to Lost Card
Please follow below process:
1. Pay Php 200.00 for replacement fee through bank deposit or transfer:

- Send clear copies and details of the following via a ticket.
- Deposit slip or proof of payment of the replacement fee
- For whom, member or dependent, is the card replacement request for
- Once complete, this will be endorsed to the provider for their processing
- Updates will be sent when:
- Provider has clarification regarding the application
- Provider confirmed approval of the request and is for processing
- Once card is available for claiming - employee will receive notice when they can drop by the office for the pick up the HMO card
B. If due to correction of wrong details
Please follow below process:
1. Pay Php 50.00 for replacement fee through bank deposit or transfer:

Account name: COCOLIFE
Account number: BDO 0054-2001-1418
- Send scanned copies of the following via a ticket.
- Deposit slip or proof of payment of the replacement fee
- Employee Name
- Dependent Name (if applicable)
- Wrong detail/s in HMO Card
- Correct detail/s to be printed in the New HMO card
- Once complete, this will be endorsed to the provider for their processing
- Updates will be sent when:
- Provider has clarification regarding the application
- Provider confirmed approval of the request and is for processing
- Once card is available for claiming - employee will receive notice when they can drop by the office for the pick up the HMO card
*Please note also that the card replacement/lost ID will only be released upon receipt of the required fee.
