FAQs

A. When can I file for reimbursement?

  1. Reimbursement applications should be submitted within 30 days from the date of availment.

B. What are the requirements for reimbursement?

  1. Filled out Intellicare reimbursement form (see attached file for reference)
  2. Original Official Receipt (with TIN)
  3. Statement of Account from the Hospital
  4. Medical Certificate
  5. Laboratory results (if with diagnostic procedure)
  6. Operative record with histopath (if with operation)
  7. Police report & Medico-legal Report (if required)

C. Turnaround Time

  1. Processing for review and approval is at least 20 working days from date of endorsement
  2. Further notices will be sent to the employee depending on the feedback from the provider

D. Reminders:

  1. Members are to register in the Agora website or mobile application to file for reimbursement.
  2. Members are to keep safe all original related documents to the reimbursement application for future reference of Intellicare as needed.
  3. The following are not covered and cannot be applied for reimbursement:
    1. Prescribed medicines from out patient transactions
    2. Prescribed eye glasses/contact lenses
    3. Dental works/procedures considered as surgeries (ex. wisdom tooth extraction)
    4. Any treatment which are not recommended and performed by a Physician as being medically necessary, including any charges for non-medical services such as:
      1. telephone, radio, television
      2. extra bed, extra food
      3. toilet articles and the like
      4. private duty nurse or physician during admission/in patient cases

E. Who can I ask for questions/clarifications regarding the reimbursement application?

  1. Please initiate a ticket via Zoho Helpdesk to People Operations or;
  2. Send an email to wellness@infinit-o.com or hr@infinit-o.com
