Please obtain the following documents:
1. SSS Sickness Reimbursement Form – Print form, to be filled out and signed by the employee (please write legibly and strictly no erasures).
2. SSS Medical Certificate Form - Print form, to be filled out and signed by the member's attending physician.
3. Photocopy of 2 valid IDs both with signature and at least one (1) with photo and date of birth, submit any of the following:
4. Medical documents related to the filed sickness notification:
- laboratory results related to sickness/diagnosis
- medical certificate
- medical abstract
- operation record
- discharge record
5. Once documents have been prepared and complete, send soft copies of all related documents via ticket to HR-ER for checking.
*Note: All physical and original documents are to be kept safe by the member for future reference and in case SSS would require submission of the physical/original documents to their office.
6. Upon receipt of soft copies of the requirements, the sickness reimbursement will be filed thru the SSS portal.
7. A copy of the transaction number will be provided to the member for reference.
