Sickness Notification FAQs
A. How will I know if I am eligible to apply for the sickness benefit?
- If you were unable to work due to sickness or injury and confined either in a hospital or at home for at least four (4) days;
- If you have paid at least three (3) months of contributions within the 12-month period immediately before the semester of sickness or injury;
- If you used up all current company sick leaves with pay; and
- If you notified the employer or the SSS, if unemployed, voluntary or self-employed member regarding your sickness or injury.
B. When should I notify about my sickness for me to avail of the SSS sickness benefit?
- Member should notify the employer within five (5) calendar days after the start of sickness or injury with the requirements to avoid late endorsement to SSS and possible disapproval
C. I would like to have my sickness notification done online while completing the physical documents, what details do I need to provide?
- Please provide the following details:
a. Employee’s SSS Number
b. Full Name
c. Start of sick leave
d. Confinement Type: (Home or Hospital)
e. Number of days applied: (as recommended by Doctor based on Medical Certificate, based on filled out Sickness Notification form)
f. Doctor’s PRC/ID No.: (License Number of Attending Physician)
