Filing for Maternity Notification
Please obtain the following documents:
1. Maternity Notification Form
2. Allocation Form
The Maternity Notification Form and SSS Allocation Forms are attached to this page.
3. Original copy of Ultrasound or Obstetrics Report
4. Photocopy of 2 valid IDs both with signature and at least one (1) with photo and date of
birth), from the following below:
5. Once documents have been prepared and complete, send soft copies of the documents as a ticket for checking and confirmation from HR
6. All original physical documents must be kept safe by the employee for future reference or in case it will be required by SSS for clarification.
Online Maternity Notification
1. For us to send the online notification, please provide the following details:
a. SSS Number
b. Full Name
c. Expected Delivery Date
d. Allocation of maternity leave credit, indicate if:
1. Employee will allocate, provide the following details:
- No. of days to be allocated (maximum of 7 days only)
- Indicate if allocation of leave is for child's father or alternate caregiver
- Full name of child’s father or alternate caregiver: (with middle name and extension name if any)
2. Employee will not allocate leave credits and will avail the full 105 days on her own
e. Ultrasound report with indicated expected due date
2. Send all requirements as a ticket.
3. An email confirmation will be sent with the copy of the SSS online notification for your reference.
