  2. Provide the following details:

    2. Birth date:
    3. PHIC number:
  3. This will be forwarded to Philhealth for them to provide the updated MDR
  4. Once received, it will be sent back to the member for their reference

For the request:

  2. A 12-month period of the contribution will be provided and sent for your reference
