Pag-Ibig Loan (Calamity or Multi-Purpose Loan)

Please obtain the following documents:

A. Multi Purpose or Calamity Loan Form
  1. Calamity Loan Application Form
  2. MultiPurpose Loan Application Form
    1. Declaration Form
B. Print form, front page and back pages (Certificate of Net Pay) are to be filled out and signed by the employee and 2 witnesses (please write legibly and strictly no erasures)
  1. Also fill out in the form the below details:
    1. Head of Office or Authorized Signatory: Roland Daniel Ong
    2. Designation: VP-Finance and Administration
    3. Pag-Ibig Employer ID No: 8001-7074-3

C. Once filled out, scan the employee signed form and send the scanned copy as a ticket for endorsement and signing by the authorized signatory
  1. Please be advised that turnaround time for the signing of the forms may take 1-2 weeks, depending on the availability and date of visit of out authorized signatory in the office

D. The employer’s signed copy of loan form will be sent back to the employee

E. Prepare one (1) valid ID
  1. Photocopy or scanned

F. Proceed to the closest or visit any of the Pag-Ibig branches to submit your loan application
*Note: There may be branches of Pag-ibig that would require the physical signed form for the loan application to be processed. For such cases, kindly coordinate with our SSP Team to either pick up in the office the signed form or have it delivered via applicable courier services with their assistance. Certificate of Contribution

For the request:
  1. Please send a ticket and indicate the purpose for the request
  2. A 12-month period of the contribution will be provided and sent for your reference
